Normal supported files now work fine

Unfortunately transcoded ones can't FF/RW and they are still out of sync

I have attached the log - I am logging at ALL level so it is a bit big

Do you have "HTTP Engine v2" enabled?
Please post also pms.conf file and MediaInfo of your file. A/V sync problem can be due to "bad" PMS setting (Alternate A/V sync ON/OFF), number of CPUs used etc...
...or if your audio is AAC. I almost never spotted A/V sync issues with AC3 (I have remux AC3 ticked) but read lot of complains when someone had audio track with AAC.

I will try converting another file to AC3 (most of mine are AAC) the only one I have is 11gb and that stutters with the current config file you have given me.

Your pms.conf are quite empty

Weird about your PMS exception in log. What regional settings do you have? Mandatory is if your settings uses "." or "," as a digit separator.
Also try to update your Java version to the latest one. I suppose you are using 1.7.0?
EDIT:
Now I see you are using v1.72 of PMS? In log I saw 1.71, GUI shows 1.72.
Try to use v1.71 to be on the same boat with me.

Sorry I updated to 1.72 to see if it fixed the issue - I'll downgrade to 1.71 (plus update java) and post a new log.

Ok - so now running 1.71, latest JAVA, performed a test with the same video but different audio - one using AAC and one AC3 - you guessed it the AC3 one is in sync.

Please don't use PS3 when you are testing Panasonic. It makes garbage from your log.

As you can see, MEncoder tries to seek 91107sec position, it means 25hours. Something weird on your side.
I never tested MP4 file, do you have some MKV file with AC3? I only tested this kind of files.

I'm not using a PS3 when testing, I think this is because the conf file you gave me has the PS3 as the renderer.

I will check with an MKV. I don't know why the video is jumping so far ahead, with non transcoded files it works perfectly.

You are right, I forgot it 8-)

I spotted "User-Agent: Panasonic MIL DLNA CP" so it cheated me it was detected as Panasonic so you have also other PS3 renderer.
If possible try some MKV file and it will be great to upload somewhere short sample of your mp4 file which generates misleading timeseek positions so I can test it when I will be at home during the weekend.
